We work across three sites.
Ashgate, our main site is a modern, purpose built premises on the edge of Chesterfield town centre.  This building also houses the 111 and GP out of hours service, along with a nursery and a café.

Holme Hall is our smallest site and is staffed by one GP (on a rolling rota) and one reception/admin team member.  This site also houses our weekly physiotherapy service, phlebotomy service and nursing services.

Whittington has 4 downstairs consulting rooms and one clinic room on the first floor.  This is again a purpose built site and is situated about 4 miles from the town centre.

Our current list size is 14,180.

We have an excellent nursing team comprising of three practice nurses (two of whom have recently been recruited and are due to commence in October 2016) and the third, a very experienced Nurse who provides support for the whole team.  We also have one Community Matron and the four Nurse Practitioners as mentioned above.  They are supported by two Health Care Assistants and a phlebotomist.

Currently our district nursing team is based at Ashgate Manor.

We have extended hours surgeries one morning and one evenings per week. 

Regular visitors to our sites include the Aortic Aneurism Screening Service, Derbyshire Live Life Better service, Citizens Advice Bureau, Physiotherapy, Diabetic Chiropody amongst others.

Our practice population is fairly traditional with many extended families, ex-miners and a very small ethnic minority.

We care for the residents of two local nursing homes.

Our practice population has increased significantly over the past two years due to our new town centre facility at Ashgate and also in response to the restructuring of a very large Chesterfield Practice which was taken over by the local hospital.

We have a balanced, pleasant and rewarding range of patients to work with.

There is a high prevalence of chronic disease.

High priority is placed on excellent clinical care and always strive for 100% QOF achievement with lower than average exception rating.

Great emphasis is placed on ownership, staff development, team working and support.

We hold various regular meetings:
Nurse team meetings and clinical meetings alternate weekly
Monthly full team QUEST (Quality U E Staff Training – protected half day training) at these we get the opportunity to discuss practice issues, host outside speakers, run in-house teaching and present recent audits.

Monthly Partners meetings with a half hour slot for Lead Nurse, Lead Salaried GP and Lead Admin/Reception feedback
Weekly Practice Director and Practice Manager meetings
Weekly Team Leader (admin/reception/premises) meetings

We also meet for coffee each morning between 11 and 11.30

The practice uses TPP SystmOne and has made highly effective use of our IT.

We have electronic prescribing, online appointment booking and online access to medical records. Our pathology tests are ordered and received electronically.

The Practice Manager, Paula Elliott, oversees the running of the practice with the support of the Team Leaders and will be able to direct you for any particular issues you may need to discuss.

The Practice Director, Kate Chilton, holds a business and strategic role but will also always be happy to meet with you to discuss any issues you may have.

Avenue House and Hasland Partnership is based in central Chesterfield and serves 13,500 patients. We have 6 GP partners (2 of whom are trainers) and currently employ 3 salaried GPs.

The partnership has two sites although GP registrars are currently based in the town centre at Avenue House Surgery. We also offer placements for third year medical students from Sheffield Medical School.

The Practice strives to be progressive and innovative, whilst maintaining the core ethos of continuity of care and family care. We like to offer a supportive environment for learning and pride ourselves on a ‘practice wide’ approach to training within a positive and friendly atmosphere. Our registrars tell us that they feel well supported with easy access to their trainers. We hold daily ‘debrief’ sessions and weekly tutorials, tailored to individual needs.

The Practice has an excellent Primary Health Care Team including Nurse Practitioners, Practice Nurses, Health Care Assistants and an in house Phlebotomist. Working very closely with the practice staff, are the community staff, including District Nurses, Health Visitors and Midwife. The Practice is supported by our experienced team of administrative and reception staff. The team meets as regularly as possible and has a proactive approach to improving patient care.

In addition to a range of GP appointments being offered (pre-bookable and book on the day), we also offer Nurse Practitioner appointments plus a range of chronic disease management clinics, run largely by the nursing team. We have access to welfare rights advice, citizens’ advice and counselling services.

Our Practice is a high achiever in the Quality and Outcomes Framework and also achieves well under patient access/appointment targets and in patient questionnaires.

The Practice offers extended hours to patients, as well as participating in a range of clinical enhanced services. We take an active role in Practice Based Commissioning and are members of the Chesterfield Consortium of local practices.

HISTORY, DEVELOPMENT AND IDENTITY OF THE PRACTICE

The practice first opened its doors on 1st June 1984 as a single handed-practice with a list size of nearly 1500. It has developed and continued to grow and is currently responsible for the health care of just under 3000 patients and is accountable to Derbyshire County PCT. The practice is a PMS practice and prides itself in being at the forefront of service development and delivery.

There are currently three practices in Bolsover serving an estimated population of 16,000.

The practice essentially serves Bolsover and surrounding local villages of Carr Vale, Duckmanton, Hillstown, Palterton and Scarcliffe.

Practice has a high proportion of elderly with patients in three nursing homes and one residential home.

The practice also has sole care of service uses in the local ABI (acquired brain injury) unit who cater for approximately twelve residents.

Bolsover is proud of its castle standing at the top of the hill, on main road into Bolsover from Chesterfield (A632). It has developed into a bustling, historic market town, easily accessed from M1 motorway junctions 29 & 29A.

The Peak District National Park, Chatsworth House and other places of interest are ten to fifteen miles west of Bolsover.

Bolsover is a former mining village, suffering higher than desirable unemployment levels. There are health consequences for ex-miners, mainly in the form of breathing difficulties plus musculoskeletal problems in later life. A Chemical Refinery in use until 2004, impacted on patients’ health in the form of breathing issues.

Bolsover has its fair share of problems related to socio-economic circumstance, among them drugs, anti-social behaviour and alcohol abuse.

The practice does no outside private work.

PRACTICE PREMISES

Practice operates from single storey premises, purpose built in 1987 with extensions added 1994 and 2013 to allow additional in-house services proposed at the time. Practice is currently being further extended to accommodate two extra consulting rooms. Castle Street practice occupies the site of a former Presbyterian chapel and is built from it’s original stone. One half of ground to front and side is still consecrated land.

The practice has a full complement of staff, including Practice Nurse and Health Care Assistant.

Practice Manager is Mrs Karen Bestwick.

ATTACHED STAFF

District Nursing Staff based at Bolsover Clinic are shared with one of two other practices in Bolsover.

Practice also shares a Midwife. A Health Visitor is solely attached to the practice. Phlebotomists are based at Bolsover Clinic and work for all three of the Bolsover practices. Community Matron is shared with the third practice.

The practice offers many clinics and services for population it serves. It prides itself on offering in-house physiotherapy, counselling, welfare rights advice, debt counselling and dietetic services.

COMPUTER SYSTEM

The practice is paper light and currently operates Systemone.

ON-CALL / OUT OF HOURS

Out of hours service is provided by Derbyshire Health United from 6.30pm week days and during weekends. Service is commissioned by the PCT on behalf of practice.

We are an enthusiastic and friendly training practice made up of 3 Partners and 4 Salaried Doctors. All have an ethos of providing high quality personal medical care to the patients but at the same time value our own quality of life.

The practice is based in a new purpose built medical centre opened in June 2008, situated in the centre of the small town of Dronfield on the edge of the Peak District. Dronfield is predominantly middle class and serves as dormitory town primarily to Sheffield and Chesterfield.

The practice population is generally mobile, well educated and receptive to preventative medicine, as a result the practice consistently achieves all targets. However, because the patients are so well informed this does create a high demand on our services. The practice population is relatively stable, at just over 9800, but we do have a fairly rapid turnover of young families who move in and out of the area as employment dictates. However, recently we have noted an increase in our elderly population and the number of retired families.
