We are a five partner PMS practice with 11600 patients. The Practice currently employs 5 salaried GPs who work a variety of sessions. The practice has been attached to the Chesterfield GP Specialty Training Scheme for many years, and is keen to build on and develop its skills in this area.
The surgery is located in modern, well kept and equipped, purpose built premises, in the middle of a mixed residential area. The surgery is situated on the northwest edge of Chesterfield.
The practice has a good mix of patients from all backgrounds, although unemployment and non owner-occupier rates are higher here than in surrounding wards. Although predominately urban, we extend out to some rural villages on the edge of the Peak District. The surgery itself is just 10 minutes drive to the M1 and has good road and rail links to Sheffield, Nottingham and Derby.
The practice has a Primary Health Care Team of 40+; this includes Nurse Prescribers, Practice Nurses, Health Care Assistants, Phlebotomists, attached Health Visitors, District Nurses, Midwife, Physiotherapist, and full administrative support.
The surgery is open each weekday between 8.00am – 6.30pm and provides an extend hours service to 8.00pm on Tuesday and Wednesday evenings. The surgery operates a 48/24 hour access system with 10 minute consultation slots - patients can book on the day to be seen. Patients are also able to book some appointments up to 4 weeks in advance.
This means that each morning and afternoon session, doctors have approximately 13 available slots, and any overspills are shared equally amongst those on duty. The number of home visits, does of course vary each day, but typically averages one to two per doctor. We operate a personal list system, but this is flexible to allow for an even distribution of workload.
The Practice achieves high QOF scores, participates in national and locally enhanced services, and does well in patient surveys.
The Practice Nursing Team run a variety of disease management clinics, and everyone on the clinical team is involved in audit, developing protocols, recalls and the implementation of NSFs, NICE Guidelines + PCT protocols.
The partnership meets informally each day with other members of the clinical team and more formally, once a month with the wider team, to discuss and review practice business. At present each Partner has responsibility for particular QOF indicators, clinical areas and also holds a non clinical role, with a brief for areas such as staffing, finance, training, IT and CCG.
The administration and reception team are cheerful, efficient and compliment the delivery of clinical care, dealing effectively and politely with enquires from patients, health professionals and other agencies. The practice uses the System one clinical software system, and is at present paper light. GPs also have access to electronic scanned workflows + lab links. We have developed a practice intranet to try and stem the tide of paper. The Practice also participates in choose and book, which has helped to reduce the amount of paper, now all letters are sent electronically.
Chesterfield is well served by a modern district hospital, and relationships with local consultants are excellent. We are on good terms with other local GPs and are part of the Chesterfield CCG & Dr Upendra Bhatia is CCG lead.

Semi rural practice with a list Size 8,815. 4 equivalent whole time GPs. 4 Partners, 3 salaried GPs, 2 Practice Nurses, 1 Specialist Nurse, 1 HCA. 14 management, reception and administrative support staff, working from a new purpose built Primary Care Centre that includes community nursing, health visitors, dental, chiropody and family planning clinic. Also in the building are Social Services and Learning Disabilities. There is a chemist attached.
